To request your records:
1. Submit a written request by email to hello@honeaowens.com. For questions about this process, please contact your therapist or the practice for clarification. No verbal requests can be accepted for medical records requests.
2. Include your full name, date of birth, and the specific records you are requesting.
3. Requests may be subject to identity verification and applicable fees as permitted by law.
4. Records will be provided within the timeframe required by Texas law.

If records are requested on behalf of another individual, documentation verifying legal authority is required. Examples include guardianship documentation or medical power of attorney.
Identity verification may be required to protect client privacy and confidentiality in accordance with applicable state and federal laws.
5. Any records requests will be released to the client, directly, via a secure and HIPAA-compliant portal message.
We do not release records to third-party recipients without proper authorization verifying legal authority. This applies to releasing records to therapists the client is seeing. The records will be released to the client who will have the responsibility to deliver the records to their therapist in a manner they choose.
The practice does not have a method to deliver records to anyone besides the client or the client's legal representative in a HIPAA-compliant and secure manner.
